Medium Term Accommodation

We provide an MTA housing and support package for clients with complex care needs as well as those looking for active support to improve participation, choice, and quality of life ensuring that they are supported in all aspects of daily living, including access to appointments, attending cultural or family events and developing and maintaining social relationships.

We collaborate with you to develop an individualised support plan prior to MTA intake to ensure:

You have opportunities to improve health, wellbeing, and continued independence development

N

We include a person-centric support plan

N

The plan which is targeting to work towards the goals agreed with you and your therapy team

We offer Medium Term Accommodation whilst you are waiting for your long-term housing solution which may include:

N

leaving hospital, Aged Care or a custodial setting

N

experiencing a breakdown of informal supports (eg.family/carers) and you can’t live in your current home

N

waiting for home modifications to be completed

N

waiting for new SDA to be built

N

waiting for a vacancy to become available in a SDA where you want to live and an offer has been made

Housing accepting MTA funding is usually available for upto 90 days.
